My child is in danger
My ex and I have a 15 month old child together. Ever since she was born he has not paid any medical bills or contributed more than 50 dollars a month towards her. When we were together I took photos of his home with drugs and alcohol bottles all over it. I also have hospital records showing he was treated for alcohol intoxication. He also received a DWAI, was fired from his job for refusing to take a drug test, arrested at his mothers home to be taken to detox. He also has a previous history including domestic violence and served 8.5 months in jail for do to random offences. I have also documented when he was going pick up our child and did not show up. He is also dating a women with a past history for using drugs. I am afraid if he gets custody of her she will be hurt in some way. Do I have enough evidence to stand a chance in court?

Yes. You should initiate a child support action with the district court where you and the child live. You should also ask the court to adopt a parenting time plan that gives you sole custody. When you do this, explain to the court why you are asking for sole custody. You should propose that the father's parenting time, if any, should be supervised.
