Guardianship of minor child

My 16 year old son has moved 300 miles away to live with my oldest son who is 22. Do I have to go through the court system to give my oldest son authority or a guardianship so he will be able to put my 16 year old on his insurance, sign for him to have medical attention, register for school, etc., or can I sign a simple non-court filed affidavit or something attesting to my permission allowing my oldest son authority over my younger son? I currently have sole custody of my 16 year old son.

You can give your older son a power of attorney for guardianship to accomplish what you want.
