POA for the care of children

how do I fill out POA for the care of children when we live in different states? The paperwork provides no area for 2 separate addresses and it also needs to be notarized. We are giving POA to our 26 yr.old son, for the care of our 16 yr. old daughter. Do I need to fill out two separate forms, one for me and one for him?

This appears to be a duplicate question. However, it might be easiest to fill out two sets. To the extent the form you have has room for all of the necessary information, you can have one set of papers with two signature pages. This is assuming that the signature page is the only place where there is not room for all of the necessary information. Otherwise, you will need two complete sets.
