power of attorney for the care of children

I need to know how to fill out this form when the parents don't live together. We live in different states, and are giving over parental authority to a relative while our child goes to school. The paperwork wants one address and one notary seal. Can I give the relative two sets of paperwork, each one filled out by one parent?

You can give the relative two sets of paperwork, or one set of paperwork with two sets of the signature page(s). Together, it will operate as the total authority paperwork.
