Out of Wedlock children - custody, residency

I have 2 small children (ages 1 and 2) out of wedlock. I have been living with my girlfriend for several years, and support my family financially. Mom stays home to take care of children, etc. She has recently broke things off with me after meeting another man. She has threatened to take the kids and leave the state. We have lived in this state for the past 2 years. Can she take the children out of state without my consent? I am the father on their birth certificates, but we have no other legal documents. I am willing to work out visitation/support, etc. but I don't want her to leave the state with my kids. What should I do?

You need to file for allocation of parental responsibilities immediately. She can take the child and leave the state without your consent until there are court orders that specify where the child is to be. File immediately and it will ground the case in Colorado. Then you can get court orders that specify your parenting time and the decision making authority.
