How can I evict my tenant quickly?

I own a townhouse in CO. My tenant has defaulted on his lease agreement several times. Attempts to reach him are unsuccessful (email, phone, personal visits, certified letter). He previously signed a 6 month lease. When that agreement expired, he signed a month to month. He is impeding the sale of the home and I want to terminate his lease but can not reach him to notify him. What's the best legal course I can take? According to his agreement, I only have to give him 30 days notice.

Without knowing Colorado law, I would give him the 30 days notice in writing and send it to the address in the lease, or the property address if none specifically set out for notices. Then, if the tenant remains after the 30th day, you can evict.
