I need help writing an Article 138. What is the format? How long does the command have to respond?

Re: Article 138
You could write an article 138 on a napkin and submit it. There is no set format. However, as a general rule, I would recommend the following format:
I. Nature of Grievance
II. Steps Taken In Chain of Command to Deal with Grievance
III. Steps taken by Commander on the grievance
IV. Why Additional Steps are Needed.
V. Request for Relief
Check out the UCMJ or ask JAG how long the command has to answer your Article 138.
