Legal Question in Credit and Debt Law in Connecticut

I saw a physician in 1/08 who did not submit a claim to the insurance co. I have since moved (with a forwarding address) and this has gone to a collection agency. They state that I am responsible for this bill, have threatened to ruin my credit if I don't pay. The insurance co is willing to pay, since the claim was never submitted, but the collectors state that it is too late and I need to pay. Am I responsible?

Submit the bill to the insurance company and just let them pay it. (the collectors don't want you to submit it to the insurance company because they want to take their money out of the payment. However, they can get paid by the doctor)
