401k Minor Child Beneficiary

My wife recently passed away, and

my 16-year old son was named as

one of the 401k beneficiaries. In the

paperwork, it states that in order for

a minor child to collect the money, I

must include a copy of the

document(s) authorizing me to act

on his behalf. What are these

documents, and where would I get

them?

It depends on the amount he'll receive from the 401K. If it's more than $10,000, you'll need to go to the probate court and get appointed his gaurdian. If less, you don't need to be appointed, but you'll have to give the administrator of the 401K fund a copy of the relevant statute which says that under $10,000 you don't need to be appointed.
