Adding father to birth certificate and changing childs last name

Is there a time limit as to adding the father to a birth certificate? The child is now 8 years old father has been actively involved since she was 2 but b/c of problems when baby was born he wasnt put on the birth certificate or given his last name. Does this have to go through the court or can it be done right at the town hall?

I believe you can do this through the state. There is actually a form you fill out and I believe you both sign it to change the child's name on his/her birth certificate.

I know they have the forms at the Superior Court but I would think they have them at vital statistics. There is a small fee and the form is filed with the state not the court.

You may want to look online to see how the process works. As long as you are in agreement I do not believe you need a court order and I don't believe there is a time limit as far as I know.
