Agreement for non-married couple
I am currently living with another person (5 years) and am not marrier nor do we have intentions of getting married in the near future. Is there some type of stndard agreement form that can be written up that basically states that all assets and debts (current and future) will remain the responsibility of the person they are assigned to and that the other party has no rights or claims to them now or in the future?

Yes, you can enter into a contract between the two of you to agree as to claims and responsibilities. It should be specific, though. It might cost a few hundred dollars to get a lawyer to do it, but it's a lot safer to do it that way.
