child custody and name change

I would like to know how to keep my child's father away from her. He has a sordid past and a criminal history. I want to raise my daughter on my own and he's just been released. Since he and I never married I want to change my daughters last name as well. I don't want him in my life? How do I make this happen?

Depending on whether you already have a custody order in place, you would have to file for sole custody. In addition, you could also apply for a termination of parental rights.
