Re: Child medical decisions

I don't believe there is a printed form that can be used for all purposes that may arise in caring for children. However, a form can be drafted to give you powere to arrange for medical treatment. But, your question does not state the status of the father. He has rights, unless they have be terminated, and his opinion and agreement may have to be included in any documents that are signed. More information is required to give a complete answer. Yes, there is documents that can be prepared to give a care taker right to authorize emergency treatment, which may have to be given anyway by a hospital, but the question is who has the power to give those rights to the caretaker. I would advise you contact an attorney and explain the situation, relationships, etc. and have a form drafted to protected to the children. Very truly yours, Thomas F. Noonan
