In the state of Connecticut, can a single woman who gives birth, put the fathers last name on the birth certificate as the new born child's last name?

Yes, so long as the mother and the father complete the Acknowledgement of Paternity Form. This form can be completed at the hospital or at a later date at any office of the Department of Social Services or Department of Public Health. Otherwise, you will need a court order.
