? duty to disclose settlement to insurer

If a client in Massachusetts receives a settlement from the other party for personal injuries sustained in RI do they have to notify their Connecticut insurer of the amount? What if the insurance co. has a lien on the settlement?

I think the RI and CT aspects of this are irrelevant and this is a Mass. law question, by the way.

Is this a purely academic question or not? Was there litigation and did the insurer put a 251 lien on the case? I assume the insurer already settled up with their client and THEN the other party paid that same client additional money, and you want to know if that client has a duty to pay back the money or even to inform the insurance company of receipt of the funds. Is this right?
