Is there a form I can use to add the name of my spouse to the deed/title on my house? Thank you.

Sure. All you need is a quit-claim deed and a conveyance tax form. You can get the tax form from the town clerk. The quit-claim you might be able to find in a form book or at a stationery store. We can do it all for you for $75 plus $43 for the recording fee to the town.
