Executing A Release of Mortgage

How would I go about executing a release of a mortgage I hold as a lien for an outstanding debt that has been satisfied?

There's a simple form obtainable at any legal forms store (maybe at Staples, for all I know)which has a place to insert your name, the mortgagor's name, and most importantly, the volume and page where the mortgage is recorded. It must be signed by you, witnessed by two persons and notarized (the notary can be one of the witnesses). You have it executed and give it to the mortgagor who records it at town hall ($13 fee).
