Legal Question in Bankruptcy in Delaware

Creditor Filing

I work for a catering business who did business with a company who filed bankruptcy in Delaware. We need to file a Proof of Claim form with the Bankruptcy Court, however, I am unable to locate one. How can I obtain a form to file with the court? Thank you.
