Alternate Power of Attorney (POA)

I purchased your Durable Power of Attorney form for the Washington, D.C. Area. I would like to name an alternate POA. I know that it can be done via a lawyer's office, but can it be done on your form? Can I just annotate that information?

If the POA form is executed in accordance with the requirements of the particular jurisdiction for which it is intended to be used, there should be no problem.
