Signing over custody

When signing over custody of a child, who would handle it and would both parents have to be present? So we both simply have to sign a form, what is the process?

Parents of a child cannot transfer custody of a child simply by signing a form. Such a transfer

of custody to be valid must be handled through a court having proper jurisdiction over such a matter with a valid order signed by a judge of that court.
