Re: Small clams

Hi,

I would suggest taking a look at some sample affidavits to get the wording that you're looking for in your case.

Here's some sample language:

AFFIDAVIT OF JOHN FRANCIS DOE

I, John Francis DOE of 26 Little Street, Browns Plains in the State of New York, being

duly sworn, make oath and say as follows:

1. I worked for Company X for 10 years;

2. My job duties included .....;

3. I received a salary of ....;

4. On May 5, 2002, I gave notice of resignation to my supervisor;

5. My supervisor informed me that my last check would be mailed to me in two week on May 17, 2002;

6. When I received the check the amount was incorrect and short XXX dollars;

7. I contacted my employer to get the matter corrected but received no response;

8. ...........

You'll want to sign and date the affidavit.
