health insurance on minor child
Former wife is responsible for health insurance on minor child. Former wife responsible for all co-pays. uncovered resonable expenses 50/50 split. Father (thats me) shall be informed of all major medical decisions and reserves right for a second opinion within 14 days before determining cost is reasonable. 1) does former wife have to supply me with a hippa form? 2) is dental part of the health insurance? example braces 3) former wife has present husband supply the health insurance card, and she now states he does not want to sign hippa forms to the health insurance company or health care practioners. 4) former wife is sending me medication billings stating I need to pay half. Well these bills are co payments but she keeps on sending them plus dental bills for cavities at this time which looks to me like a family deductible due to a policy folder she sent me stating thats all I get and pay the bills she sends me.

HIPPA forms should be signed by the primary residental parent typically. They are necessary to treatment. If your agreement/order says that Wife is responsibile for co-pays then you need to follow the letter of the agreement. No, dental/braces is not usually included in medical unless your agreement states otherwise. I would think that uncovered medical would include the prescription costs however.
