Your question is really broad. Think of it kind of like asking how you fill out an IRS Form 1040 tax return and its supporting schedules.
Just like there is an Internal Revenue Code which is ridiculously long and complicated and subject to the whims of Congress every year, there is a Bankruptcy Code that is ridiculously long and complicated and subject to the whims of Congress every year.
It sounds like you need some help with this. Self-representation may not be for you. It involves a lot of independent research. Sometimes it's really just worth it to pay the lawyer to take the headache.

Your question requires more than can be posted in a forum such as this. While the law allows for pro se bankruptcies many people find the process confusing and difficult to navigate.
