I am filing pro se bankruptcy in northern florida and I would like to know if it is ok to write side notes on my forms or can I add attachments to forms and if so, does it need to be noted on the original form?

You should not make notes to your forms or include attachments. The forms are standardized and should include all the information for your bankruptcy. You should speak with a lawyer if you feel the forms do not meet your needs.
