What type of legal form would I need? I have an idea for a company to make a specific item to include in their inventory and distribute to all of their locations. However I want to be paid for the idea. What's the first step in this situation?

You should probably discuss your idea with a patent lawyer. Nothing is ever as simple as just a "form."
