Operating Agreement
If an LLC is member managed with one member only, does it still require an operating agreement and who would the operating agreement be between since he's the sole member?

You should still have one even if a single member. The parties would be the member and the company, signed by the member on behalf of the company and personally. You should also keep minutes of meetings.
