Release form for Photos Used in Catalogue
Someone who has purchased my products in another State (my business is in Florida) has offered to send me photos of my products being used by her clients. I asked if I could use those photos in my catalogue and on my wesbite, and she asked to provide a release form for the persons being photographed or their parents or guardians. How do I go about obtaining or creating such a form?

It does not need to be anything fancy, just draw up a simple document that says that the signer is giving you permission to use the photos in the manner that you wish to use them.
