wanted to know if I could add another business description or phrase to my business without changing my business license. I have a medical clinic name already and was considering adding something like "diabetic care center" or " " a location" diabetic care center"

Probably. It depends upon where and how you do it. Possibly a Fictitious Name Registration may be needed. You have to make it clearer first to know for sure.
If you change your DBA name, you will probably need to get an updated business license with the new name. Call your local city hall (or wherever you obtained your business license) and ask them. Consult with a good business attorney in your area for more specific advice.
