An answer to this would be too long for this forum. You should see a lawyer or you can consult a multitude of forms to get a type of invoice.

RESPONSE: As far as disclosures which are legally "required" to be in your contracts, go to our website at www.theconstructionlawyers.com, click on Forms and then look at "DISCLOSURE - 713.015 MANDATORY LIEN DISCLOSURE" and also "DISCLOSURE - NOTICE OF CLAIM 558.005" Your city, county, or locality may also require other disclsoures as well. In addition, the construction licensing laws require that you have your name, address, and contact information on all invoices/contracts. There may be other disclosure requirements, but these are what I can think of off the top of my head.

Good luck,

Randall Gilbert
