Is the trial date cancelled if the judge does not disqualify himself?
The judge has not responded to a hearing date to disqualify himself and the trial is within
4 weeks. Is the trial postponed? If so, how does defendant do this?

No. The trial is not continued. Even if the judge disqualifies himself, that does not operate to continue the trial.
