Dental fraud?

I went to a dentist and asked if they used my dental insurance provider, they advised that they did. They provided an estimate for the work that was required as well as giving me the amounts that I would be responsible. I had the work done. The bill has arrived at more than twice the estimate. Speaking with my insurance provider they informed that the dentist was not a prefered provider and that much of the extra on the bill were items charged at a higher rate than they payed and there were items billed seperate that would have been included as part of the procedure were they in fact working with my insurance provider. As the dentist indicated that they used my dental insurance service, should they not be legaly bound to charging the amounts allowed by the insurance provider? Should they also be held to amounts of the estimates as they are the expert in assessing the work required?
