Legal Question in Consumer Law in Florida

Hello I was just on the site findforms.com and they have a form Florida Website Maintenance Agreement, which probably would work for me if you go to http://www.pydreams.citymax.com/rcawebsitemarketing.html you can see what we will be offering, but in a nutshell it is Web Site Marketing, doing web site optimization, SEO, SEM and web ranking along with Linking and doing monthly reports to the clients, my question is the form Florida Website Maintenance Agreement should work for my case and the other thing, is this where I can add it to my website for them to fill out and email it to me or do I email them the agreement to fill out. If I have to email them the form, can I set one on the website for there review?
