If I am making regular payments on a bill do I have to respond to a finicial sta

I'm a transplant patient of 4 years. When sugery was done I had an insurance that paid 80% left me with 20% I have had continious follow up care. My insurance changed 2years ago to only 60% So the bills incured went up fast. I recieved a letter requesting that I show my fanicial status which asked for a lot of personal information. I have been paying them $50 per month from day one.and never missed a payment. They want more money which I can't affored. Do I have to respond to this letter? I intend to pay It will just take me longer then they want me too.
