Legal Question in Criminal Law in Florida
Where and how do I file for a subpoena in my county(Palm Beach, FLA)? And also if there is a form how do I get access to it.

You have to file a lawsuit first, then ask the judge for a subpoena (file a motion or a petition depending on the case). You can't get a subpoena otherwise -- not through a lawyer, not through a form.
A subpoena is an order of the court to comply, and failure to comply will/can lead to criminal charges. You absolutely cannot get a subpoena yourself.
