is there a form that I could fill out to ask the judge for termination of my Federal probation? If so, where do I get it?

Re: Termination of Federal Probation

There is no "general" form that I know of. You can write your own motion though. You can look up the form of the motion's caption (the top part and the bottom part of motions) that is used in the court that has jurisdiction for your case and copy that (replacing the correct names and case numbers) and title it as a "Motion to Terminate Supervision" or something to that affect. Since you would be moving this before the court pro se (without an attorney) you should be given wide latitude as to the form of the motion. The body of the motion should state the grounds for the reason why you think your probation should be terminated early. (If you go to the Clerks Office you can request to see your case file. Copy a pleading that has already been filed and copy the generic parts from there.) Or you may want to ask you probation officer to make a request to the court for an early termination of your probation. Lastly, you can hire an attorney.
