On a DUI case, are the suspensions for a refusal and the civil suspension concurrent?

If you mean are the suspensions for the "criminal" DUI and the civil suspension concurrent, the answer is yes, but with a caveat. For example, if you got arrested for a first time DUI and refused on December 1st, 2011, but didn't plead guilty until February 1st, 2012then the "civil suspension" would not be over until December 1st 2012. The criminal suspension starts on February 1st but would not be complete until February 1st, 2013.
There are exceptions to this such as the judge only ordering 6 months, or if you win the DMV hearing. Talk to an attorney to be sure.
