My 99 year old active uncle rented a house, but the agent insisted I co-sign the lease. Now, he wants to relocate closer to family into an independent living facility. If we distrust his living alone so far from family (120 miles), can he just move out with the standard one month warning. Will I get stuck for the remainder of the lease?

There is no "standard." You need to look at the lease . . . and yes, you are equally liable.

If the lease is month to month, then yes, you can give 30 days notice. Read the lease to determine when the term ends.
Both co-signers can be held responsible for the full term.
