I am POA and health proxy to my mother who has dementia. She is in an assisted living facility but wants to go home. If I let her and she has an accident and gets hurt am I legally responsible?

No-but let the Doctor make a recommendation.
Do not consent yourself to letting her go home if she is competent.
If she is not competent-you should start a guardianship to protect her from herself.
