My name is Julie, I have a son who is 19 years old and he is mentally incompetent from that time he is 5 years old, now he is over 18, the doctor said he need Durable Power of Attorney form for medical reason.
Can I fill out the Durable Power of Attorney form for him. or is the other paper work that I need to fill out so that I am the one to make a decision for him.
