Appearance Releases & Liability Waivers

I want to film my friends and I doing skits similar to 'Jackass', as well as film girls doing things similar to 'Girls Gone Wild'. I haven't trademarked my company name yet, but I want to have rights to use the footage later on, possibly for commercial purposes. What forms do I need, and will the rights be given to my name specifically in the release forms, or should I put the company name before I even trademark it, or can I do both? I just would like to release my self from liability, even though the Jackass-like 'stunts' shouldn't be too dangerous. I also want to have the girls, as well as anyone else appearing on the film, to release me the right to use their likenesses. I intend to film while on a cruise ship in the Bahamas, Cayman Islands, and Mexico. Please respond promptly if possible. Thank you in advance!
