Legal Question in Entertainment Law in Florida
can you direct me to a contract that outlines an agreement, between two people who created the concept for a TV series? This would hopefully include ownership split covering expenses and income, managerial rights (sole signature required), collaborative development and right of survivorship to name a few.

You should consult an entertainment attorney to have one drafted. You will be hard pressed to find an attorney that will turn over work product of that detail. Additionally, these contracts are not forms, but are documents drafted with the specific details and facts of your situation, tailored to your specific needs.
If you find one that was written for someone esle, it will probably not meet your needs.
