nude modeling/porn legal forms

I am going to be doing nude modeling and porn. what kind of forms & contracts do I need so that I have all rights and copies to the pictures and film that I will be doing.

There are a host of legal issues related

to adult content and porn. It is not

as simple as filling out a couple of forms.
