Adding Biological Fathers name to Birth Certificate

I was going through a divorce when my son was born from another man. The divorce was finalized when my son was 2 months old and the child was not mentioned in the divorce or decree. I married my sons father and now we are trying to put his name on the birth certificate. What steps do I take and which forms do I fill out to accomplish this task? Thanks

Petition DCF to amend the birth certificate. They have a form for that. You can find it at their website.
