Adult Name Change

In Title VI Chapter 68.07 form requests residence address since birth. Do you have to state every address you lived at from birth? If not how many years back can you state?

You are supposed to give all of your residences. As a practical matter, if you make a valid attempt, no judge will hold you to that if most addresses are listed. You should go back as far as possible.
