How to apply for legal change in custody

What do I need to so to apply for a change in custody for my 3 year old who currently resides with his mother? The child has a speech problem and I have done all the leg work to get his doctors to test him and refer him to speech therapy--his mother does not take him. When he is sick with high fevers they call me to take him to the emergency room, they do not take him to the doctor. The mother is being negligent with his health issues and his speech problems. I pay my child support every month, plus an extra $50.00. i also pay for the preschool for the child every week, and the mother takes him whenever she wants to.

What steps do I take to apply for legal custody or at least partial custody, so my child can live with me at least half of the week?? SHe is also threatening to leave out of state with the child.
