Birth Certificate

I want to change the name on my son's birth certificate from my previous husband as father to my current husband. And I also want to change my son's last name to my current husband's last name. is that possible? how do I do that?

You need to file a petition for name change and a motion to amend birth certificate, but it requires the permission of both biological parents.
