board of education legal form required for admission for custody/guardianship

A dear friend of the family had a child out of wedlock when she was 17. She asked me to be her legal guardian/custodial parent which I have been since I brought her home from the hospital @ birth (she in now 5 1/2 years old). We never took care of any paperwork at the time and now need to do so. For the immediate situation, The board of education in Wellington Florida is requiring me to give them a ''signed custody or legal guardianship form'' in order for my daughter to permitted to enter Kindergarten. I have already given them her vaccination/Florida health record certificate as well as filled out the school registration forms. I have looked on line for the ''proper form'' but cannot find the correct one. I am a single parent myself and there is no ''father'' regarding the 5 1/2 year old in question. Thank you for your assistance.

They are looking for a court order.
