My only Brother is sick and needs assistance in medical advice. Who has more authority over decision making, the Son of my Brother or me?

Without a power of attorney, neither of you has authority over decision-making. If your brother is mentally competent and able, he should execute a power of attorney appointing the health care agent he prefers to have make health care decisions on his behalf.
