Legal Question in Family Law in Florida

I am the caregiver of a 16 year old child who's mother has left the state of Florida and has not called and notified me since leaving her with me in March 2013. The letter I have as caregiver expires in June 2013. She has not paid any expenses, called to check on her or spoken to me at since March. I only know she left the state because spoke has spoke to the child a month ago when her cell phone she provided stopped working. I have no address or way to contact her at this point. She does have Medicaid for her medical and dental. After June I will not be able to get her treated medically, at the dentist or register her for school in the fall. What are my rights if any? Is this abandonment? Can we apply for custody? The mother has sole custody but the father who I do have contact with is will to give me custody. Help!
