Certificate of Deposit, Name on Account

My wife's grandfather added his son/daughter-in-law's name to his 4 CD accounts. After his son died, he went to remove his son's name from the account only to find out that his own name had been removed from all 4 accounts, 3 of which had been liquidated. The 4th has yet to mature. The grandfather strongly believes that at no point did he ever agree to remove his name from the accounts. So I ask, can someone's name be removed from the account without their consent? And, is there any possible recourse to recouping the 3 liquidated CD's, and possibly reclaiming the 4th? I apologize if I butchered this explanation.
